    • Remember to turn off wipers when parking for the night (so they don’t start frozen and break)
    • Flip up wipers if ice or snow is expected in the morning.
    • Be mindful of ice in driveways and walkways, especially if they have been shoveled which can leave ice as snow melts.
    • Do not park over the sidewalk (all year), if you notice you have parked over the sidewalk, please fix it before you come in.
  • PHEV Vehicle (White Sonata), please remember to plug back in when finished, when locked the connector won’t engage/release so lock after connection, leaving overnight is okay.

Highlights and follow-up from October Live Training

Daily Logs:

When documenting the daily logs make sure the start and end times match the census. 

Under Tasks score/ Description: 

  • If the individual was present that day you will put service provided. 
  • If the individual was absent that day you will have no service provided.
  • Scoring comments: You will give a brief description of how the day went with that individual, what they participated in, how they did that day and if there were any complications. This summary does not need to be very long, just a brief description of the day.

DO’s:

Mickey Mouse arrived at the program today in a great mood. He signed up to go to Target with his peers to do some window shopping. When he returned he had lunch and then worked on his goals and participated in the Math club. Mickey Mouse then cleaned up his area before leaving for the day on transportation.

Don’ts: 

  • Went to target
  • Had Lunch
  • Goals
  • Math club
  • Went home

Score:

  • If the individual uses transportation services , you will mark service provided
  • In the comment box you will leave blank, unless they were dropped off and used transportation in the PM or was picked up during the day but used transportation in the AM. You will include the time that they were dropped off and or picked up. 
  • If transportation was not utilized you will mark no service provided and leave the comment section blank. 

Documentation on ISP’s:

When documenting ISP/Goals, please make sure you are reading the questions completely. This ensures you are answering the questions appropriately for that individual and what level of independence was accomplished. When we don’t read them thoroughly and rush through the questions you are falsely documenting, this could lead to receiving inaccurate % ‘s on how that individual is actually doing. We don’t want to set up our individuals to fail. Our jobs are to increase their independence. If you did not run a goal or the goal was not ran that day DO NOT mark it that it was completed. When this happens you should mark it N/A. 

SL Quarterly Theme: Understanding Individual objectives. To provide the best services, we must understand the perspective of those we serve and what each of their goals mean to them. We should be giving them opportunities throughout each week to be able to self reflect on their goals and give feedback on how they want to be achieving them. This should be a comfortable conversation that is had often so that if they feel they want any changes they can receive timely support to do so. Help support and empower through better understanding.

  • Salt Lake: Makayla-She is a consistent and hard worker. She is always communicative and willing to help clients with whatever they need. 
  • Tooele: Sarah A – She goes above and beyond anytime she’s needed, if that’s helping other staff during her regular shift or even after hours/on the weekends when she’s needed. 
  • Support Living: Jenn P – Fills in supported living shifts, spur of the moment. She is always willing to help out the team when asked. Thanks for your support!
  • Employment: Scott W – Works hard and is always thinking of job opportunities for those he supports. He is industrious with building relationships within the community and advocates effortlessly for our individuals.
  • Management: Michael C- Has shown exceptional leadership skills. He is always available for staff and individuals. He is admired and appreciated by many. 

Employee of the Month Nominations (October)

  • Salt Lake: Bob Brown-Always comes in with a smile and a treat for staff. Official coffee maker of ACS. Supportive to clients and always encouraging them to read and improve their social skills. 
  • Tooele: Teona F – A guardian recently called her out on how patiently she handled a very difficult situation and in doing that, she was able to keep the client calm as well. She is absolutely adored by clients, their guardians and staff.
  • Support Living: Terry has been so great with the Individuals he works with. He thinks of ways to help them improve and become more involved with their communities. Thanks for your efforts!!!
  • Employment: Stirling has demonstrated great empathy and has gained trust with many of our Individuals. He tasks every task assigned with a smile and finds ways to help our Individuals evolve. Thanks for your dedication!!!
  • Management: Teresa has demonstrated great leadership and has built a positive environment in Tooele for her staff and individuals. Thank you for your dedication and hard work!!

Employee of the Month Nominations (September)

  • Salt Lake:  Trista M. is so compassionate and caring to all individuals. Trista is always willing to assist her team members.
  • Tooele: Tina does a lot of Supported Living with our Individuals. She isn’t always physically seen, but we see her efforts and know that she is a rockstar, she is loved by the Individuals and their families.
  • Support Living: Sara Guest is always willing to step in and help with scheduling adjustments. She works hard and gives it her all with the individuals she supports.
  • Employment: Michael Hollinger has stepped up in getting Tooele set up for Employment Supports. He attends to several work locations and is always willing to take on a challenge. 
  • Management: Nicole Bor. Helps anyone and everyone without question. She is always in high spirits and brings a positive energy.

    • Tooele: Annie Z. Annie has shown great team work and dedication. Anne is passionate about the care she gives to all our individuals.  
    • Salt Lake 1: Michael C. helps all the Participants feel included and makes the outings fun to attend. He has great time management and helps out wherever needed. 
  • Salt Lake 2:Tonja is always willing to help new staff find their way, she is patient and kind when training.
  • Support Living: Liz M. is timely and accurate with all her documentation. She works hard and accepts new tasks assigned. She is eager to learn and her efforts are valued.
  • Management: Wes has been such a big help and is always willing to step up to lend a hand.  
  • Employment: Dallin has been amazing with his time management and helping out his team with covering shifts and job development. We appreciate and see his efforts for the employment team.
