Supporting Youth and Young Adults Ages 14–22
The transition from school to adulthood is one of the most important stages in a person’s life. Ability & Choice Services’ After School & Summer Transition Program provides structured, community-based support for youth and young adults with intellectual and developmental disabilities ages 14–22.
Our program helps participants build independence, develop social and life skills, explore employment opportunities, and remain actively engaged after school, during school breaks, and throughout the summer. Rather than simply providing supervision, we focus on helping each individual develop the skills needed for greater independence and success in adult life.
What We Offer
Participants attend our community-integrated Day Hubs where they can:
• Build daily living and independence skills
• Develop communication and social skills
• Participate in community outings and recreational activities
• Learn decision-making and personal responsibility
• Explore volunteer and community involvement opportunities
• Maintain routine and structure outside of school hours
• Develop confidence, friendships, and self-advocacy skills
Every program is designed around meaningful engagement, individual choice, and person-centered goals.
Preparing for Adult Life
As students approach adulthood, continued opportunities to practice real-world skills become increasingly important. Our program helps participants:
• Increase independence and self-confidence
• Strengthen social and communication abilities
• Expand community participation and inclusion
• Maintain progress during school breaks and summer months
• Develop skills that support future employment and adult services
Employment Exploration
Our South Salt Lake and Tooele locations also provide access to Ability & Choice Services’ Supported Employment team. Participants may have opportunities to explore careers, develop workplace skills, participate in vocational assessments, and receive employment-related supports as they prepare for future job opportunities.
We work in coordination with families, schools, Vocational Rehabilitation, and DSPD to help individuals achieve meaningful long-term outcomes.
Program Hours
After School Program
Monday through Friday, 2:00 PM – 6:00 PM
Summer Program
Monday through Friday, 9:00 AM – 6:00 PM
Programs are available during:
• School Year Afternoons
• Summer Break
• Winter Break
• Spring Break
Transportation Options
Transportation may be available from school, home, or designated bus stops depending on individual service plans and location.
Funding
Services are typically funded through Medicaid waiver programs administered by DSPD. Private pay options are also available.
