Ability &amp; Choice Services&#8217; After School &amp; Summer Transition Program provides structured, community-based support for youth and\n                      young adults with intellectual and developmental disabilities ages 14\u201322.<\/p>\n                    <p>Our program helps participants build independence, develop social and life skills, explore employment opportunities, and remain actively engaged after school, during school breaks, and throughout the summer. Rather than simply providing\n                      supervision, we focus on helping each individual develop the skills needed for greater independence and success in adult life.<\/p>\n                    <h4>What We Offer<\/h4>\n                    <p>Participants attend our community-integrated Day Hubs where they can:<\/p>\n                    <p>\u2022 Build daily living and independence skills\n                      <br>\u2022 Develop communication and social skills\n                      <br>\u2022 Participate in community outings and recreational activities\n                      <br>\u2022 Learn decision-making and personal responsibility\n                      <br>\u2022 Explore volunteer and community involvement opportunities\n                      <br>\u2022 Maintain routine and structure outside of school hours\n                      <br>\u2022 Develop confidence, friendships, and self-advocacy skills<\/p>\n                    <p>Every program is designed around meaningful engagement, individual choice, and person-centered goals.<\/p>\n                    <h4>Preparing for Adult Life<\/h4>\n                    <p>As students approach adulthood, continued opportunities to practice real-world skills become increasingly important. Our program helps participants:<\/p>\n                    <p>\u2022 Increase independence and self-confidence\n                      <br>\u2022 Strengthen social and communication abilities\n                      <br>\u2022 Expand community participation and inclusion\n                      <br>\u2022 Maintain progress during school breaks and summer months\n                      <br>\u2022 Develop skills that support future employment and adult services<\/p>\n                    <h4>Employment Exploration<\/h4>\n                    <p>Our South Salt Lake and Tooele locations also provide access to Ability &amp; Choice Services&#8217; Supported Employment team. Participants may have opportunities to explore careers, develop workplace skills, participate in vocational assessments,\n                      and receive employment-related supports as they prepare for future job opportunities.<\/p>\n                    <p>We work in coordination with families, schools, Vocational Rehabilitation, and DSPD to help individuals achieve meaningful long-term outcomes.<\/p>\n                    <h4>Program Hours<\/h4>\n                    <p>After School Program\n                      <br>Monday through Friday, 2:00 PM \u2013 6:00 PM<\/p>\n                    <p>Summer Program\n                      <br>Monday through Friday, 9:00 AM \u2013 6:00 PM<\/p>\n                    <h4>Programs are available during:<\/h4>\n                    <p>\u2022 School Year Afternoons\n                      <br>\u2022 Summer Break\n                      <br>\u2022 Winter Break\n                      <br>\u2022 Spring Break<\/p>\n                    <h4>Transportation Options<\/h4>\n                    <p>Transportation may be available from school, home, or designated bus stops depending on individual service plans and location.<\/p>\n                    <h4>Funding<\/h4>\n                    <p>Services are typically funded through Medicaid waiver programs administered by DSPD.
