This summary does not need to be very long, just a brief description of the day.\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">(See Example below)<\/span><\/p>\n<p><b>DO\u2019s:<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Mickey Mouse arrived at the program today in a great mood. He signed up to go to Target with his peers to do some window shopping. When he returned he had lunch and then worked on his goals and participated in the Math club. Mickey Mouse then cleaned up his area before leaving for the day on transportation.<\/span><\/p>\n<p><b>Don\u2019ts:\u00a0<\/b><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\">Went to target<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Had Lunch<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Goals<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Math club<\/span><\/li>\n<li><span style=\"font-weight: 400;\">Went home<\/span><\/li>\n<\/ul>\n<p><b>Score:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If the individual uses transportation services , you will mark service provided<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">In the comment box you will leave blank, unless they were dropped off and used transportation in the PM or was picked up during the day but used transportation in the AM. You will include the time that they were dropped off and or picked up.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If transportation was not utilized you will mark no service provided and leave the comment section blank.\u00a0<\/span><\/li>\n<\/ul>\n<p><b>Documentation on ISP\u2019s:<\/b><\/p>\n<p><span style=\"font-weight: 400;\">When documenting ISP\/Goals, please make sure you are reading the questions completely. This ensures you are answering the questions appropriately for that individual and what level of independence was accomplished. When we don\u2019t read them thoroughly and rush through the questions you are falsely documenting, this could lead to receiving inaccurate % &#8216;s on how that individual is actually doing. We don\u2019t want to set up our individuals to fail. Our jobs are to increase their independence. If you did not run a goal or the goal was not ran that day DO NOT mark it that it was completed. When this happens you should mark it N\/A.\u00a0<\/span><\/p>\n<p><b>SL Quarterly Theme: <\/b><span style=\"font-weight: 400;\">Understanding Individual objectives. To provide the best services, we must understand the perspective of those we serve and what each of their goals mean to them. We should be giving them opportunities throughout each week to be able to self reflect on their goals and give feedback on how they want to be achieving them. This should be a comfortable conversation that is had often so that if they feel they want any changes they can receive timely support to do so.
